This is the Faith SDA Church Transportation pickup request form. Please use this form to request transporation needs.

This form must be filled out and submitted no later than noon (mid day) on Friday's for Sabbath services.

For special trips/events, this request must be submitted two weeks in advance to ensure approprate review/approvals from the Church Board/Administration is provided.

Note: If you have an urgent request or, if your plans change and you no longer need pickup, please call 1-860-890-2535 or 1-860-605-8146 to inform a transportation coordinator.
